Chronic Disease Management 

According to Medicare data, a large proportion of Medicare beneficiaries ages 65 and over across the service area are burdened by multiple chronic conditions.

Health behaviors such as tobacco use, alcohol consumption, diet and exercise and obesity are often correlated with chronic health conditions. Although tobacco use and alcohol consumption varied by location, obesity is a concern across all service areas.

Highlights of Our Impact

Food & Friends, Washington, D.C. 

Food & Friends was founded in 1988 in a church basement with 20 volunteers to deliver meals to individuals dying from complications due to AIDS. Overtime, services were extended to include individuals living with cancer and other life-challenging illnesses who could benefit from specialized meals and nutrition counseling.

With a $12,500 grant provided by Impact1890 for the Critically Ill Seniors program, 832 seniors received either home-delivered meals or Groceries-to-Go, which consists of grocery items, fresh produce and frozen meals. Clients could also participate in one-on-one consultations and cooking classes to better manage eating habits and behaviors. Read More

Jefferson Area Board for Aging (JABA), Charlottesville, Va. 

The mission of Jefferson Area Board for Aging (JABA) is to promote, establish and preserve sustainable communities for healthy aging. The primary goal is to provide services and information so older adults can live health, safe and independent lives for as long as possible in the home of their choice.

Impact1890 awarded JABA $10,000 in 2018 to help support its Live Well, Virginia! project and in particular the Chronic Disease Self-Management Education (CDSME) program. The grant enabled JABA to recruit and train more volunteers as well as provide the needed infrastructure so that JABA can strive to double the number of CDSME participants. Read More
